Claims can be submitted by mail, email, or fax to:

To file a claim inside the US: 

1. Present your insurance ID card to the provider so that they can directly bill Global Claims Administration. 

2. Download a claim form as per your issue and submit it to the mailing address. 

3. If there is any service you have paid for, attach the itemized bill or receipts, and submit them. 

To file a claim outside the US: 

1. Complete the claim form with all necessary information. 

2. Attach the bills and receipts that you have received from the foreign provider. 

3. Please submit it to the Global Claims Administrator email. 

All claims must be appealed in writing. It may take around 30 days to get a claim processed.
