Waiver Form: The plans meet most university requirements. Please call or email us to confirm the same. After your purchase, email us your waiver forms with your certificate number to [email protected] and we will have them completed and sent to your school.
PPO Network: For charges incurred within the PPO – FirstHealth PPO for both inbound and outbound plans or at a Student Health Center , coinsurance will be waived in the Select plan.
Renewal: Coverage can be renewed up to a maximum of 4 years as long as you meet eligibility requirements. Provided there is no break in coverage, you will not be required to satisfy the deductible and coinsurance.
Cancellation: To be eligible for a full refund, the request for cancellation must be received prior to the effective date. Cancellation requests received after the effective date will be subject to the following conditions:
A $25 cancellation fee will apply.
Only premium for the unused whole months of the plan will be refunded.
Only members who have no claims are eligible for premium refund
After 60 days, no refunds are granted
