The Overseas Travel Medical Plan offered
by Health Plan Administrators, Inc., is comprehensive,
short term medical protection for global citizens
traveling outside their Home Countries.
The plan offers insurance coverage for
expenses arising from overseas medical emergencies,
evacuation, and return of mortal remains. Both US
citizens/residents and non US Citizens (non US Residents)
can apply for coverage. Protection is offered outside
your home country i.e. foreigners visiting the US
or those traveling abroad can apply.
| Policy Maximum
|
The Overseas Travel
Medical Plan provides a Policy Maximum of $1,000,000;
and for those over 80, the maximum is $10,000; |
| Deductible
|
$100, $250, $500, $1,000 or $2,500
per Certificate Period. |
| Co-insurance |
- Outside the
U.S. and Canada:
For the Certificate Period, the plan pays
100% of Eligible Expenses after the Deductible
up to the Overall Maximum Limit.
- In USA :
for non-US citizens in the US., the plan
pays 80% up to $5,000, and 100% thereafter
of Eligible Expenses after the Deductible
up to the Overall Maximum Limit.

Coverage during initial purchase
is available in 15 day or monthly increments
up to 12 months, paid in full or monthly by
automatic bank drafts or Visa, MasterCard, or
Discover credit cards.
Optional Hazardous Sports rider available.

The Overseas Travel Medical Plan
can be renewed for up to a total of 12 continuous
months. |

Trust which is insured by U.S Fire Insurance

| Claims Administrator
|
The Overseas Travel Medical Plan
is administered by the Health Plan Administrators,
Inc. based in Rockford, IL, USA |
Eligibility:
Any US citizen or resident of the US who
travels or resides anywhere outside of the US and
holds a valid US passport. Eligible dependents are
your legally married spouse and unmarried children
between 14 days and 19 years old.
Coverage becomes effective on the latest
of:

The date the Application
and correct premium is received,
-
The moment of arrival
in the country designated on the Application,
-
the Effective Date
requested on the Application, or
-
The Date the Application
is approved.
Coverage will end on the earliest of:
-
The moment You
return to Your Home Country,
-
The expiration
of 12 months from the Effective Date,
-
The date shown
on the Schedule provided by HPA,
-
The end of the
period for which the Plan Cost has been paid,
-
The date You are
no longer considered an Eligible Person, or,
-
For foreign visitors,
the Date You become a permanent resident of the
US.
Plan Benefits:
The Overseas Travel Medical Plan offers
a broad range of benefits including:

Coverage for illnesses
and accidental injuries that first manifest themselves
during the trip;

24 Hour Medical
Emergency Protection medical referral/assistance
program;

Medical evacuation
to the nearest qualified medical center or hospital;

Emergency Dental
Coverage;
-
Accidental Death
and Dismemberment coverage up to $25,000, and $5,000
for dependents;

Emergency Medical
Reunion coverage up to $10,000 and return of Minor
Children coverage up to $5,000;

Return of Mortal
Remains coverage; and,
-
Interruption of
Trip coverage up to $5,000.
Cancellation: Full refund is available
if the cancellation request is received in writing
before the start of the plan. After the policy is
in effect the Plan Cost is nonrefundable.
