Patriot International Insurance is an
travel medical insurance for US citizens travelling
outside the US.
Patriot International Medical insurance provides coverage
for hospitalization, Intensive care, Surgery, Prescription
as well as AD & D, Evacuation, Trip Interruption,
Lost luggage, Repatriation and optional Leisure as
well as Extreme Sports Rider for the adventurous travelers.
Patriot Travel Insurance offers Patriot foreign travel
medical insurance offers coverage to US citizens who
travel outside the U.S. for pleasure, business or
study and is available for up to 24 months.

- Outside
the U.S.& Canada:
After the deductible is paid, the program
pays 100% to the selected Medical Maximum.
- Inside
the US and Canada:
After the deductible is paid the program
pays 90% (if PPO Network is used) or 80%
(if PPO network is not used) of elligible
expenses up to US $5,000, then 100% up to
policy maximum.

| Coverage Options |
Coverage is available for periods
of 10 days to 12 months at any given time and
is renewable up to 24 months if the plan was
intially purchased for a minimum of 3 months.
Coverage can be purchased in a combination of
monthly and/or daily periods by paying the appropriate
plan cost. |

Eligibility:
patriot International Medical insurance
is travel insurance for US Citizens travelling outside
the United States. Individuals age 70 to 79 as of
the Certificate Effective Date are subject to a $50,000
Overall Maximum. Individuals
age 80 and over as of the Certificate Effective Date
are subject to a $10,000 Overall Maximum.

Remarks:
- PPO Network: Patriot International
Insurance does not follow any PPO Network outside
the US. You can go to any provider.
- Renewal: If the Patriot International
is purchased for a minimum of three months, coverage
may be renewed withour break in coverage for a total
of up to two years.
- Cancellation: A full refund
of premium is possible if a written request is provided
prior to the start of the plan.If written request
is received after the Effective Date of coverage,
you will be required to pay a US$25 cancellation
fee and only full month premiums will be considered
for refunds provided there are no claims filed.
