Atlas America Insurance
Atlas America is an ideal insurance for non US citizens requiring coverage in the US.
Atlas insurance provides coverage for Medical, Treatment of Complications of pregnancy for the first 26 weeks of Pregnancy, Acute onset
of a pre existing condition as well as AD & D, Trip Interruption, Lost Checked luggage, Repatriation and optional Hazardous Sport Rider for the
adventurous traveler. Atlas America visitor Insurance covers international visitors who come to the U.S. for pleasure, business or study and is available
for up to 12 months.

| Co-insurance |
- Outside the U.S.:
After the deductible is paid, the program pays 100% to the selected Medical Maximum up to the Overall maximum Limit.
- Inside the US:
After the deductible is paid the program pays 80% of the next $5,000 of Eligible Expenses after the Deductible, then 100% to the Overall
Maximum Limit. Coinsurance will be waived if expenses are incurred within the PPO and expenses are submitted to Underwriters for review and
payment directly to the provider.

| Coverage Options |
Coverage is available for periods of 5 days to 12 months. Coverage can be purchased in a combination of monthly and/or
daily periods by paying the appropriate plan cost. |

The plan can be purchased for a minimum of 5 days to a maximum of 12 months at any given time. |

Eligibility:
Only non-US Citizens who are at least 14 days of age are eligible for coverage under this plan.
Individuals age 70 to 79 as of the Certificate Effective Date are subject to a $50,000 Overall Maximum. Individuals age 80 and over as
of the Certificate Effective Date are subject to a $10,000 Overall Maximum.

Atlas America Insurance offers a broad range of benefits including:
- In-patient hospitalization, surgery, medical diagnostic expenses (up to policy maximum)
- Out-patient doctor office expenses (up to policy maximum)
- Prescription Drugs (up to policy maximum)
- Emergency Medical Evacuation(up to $500,000)
- Political Evacuation (up to $10,000 Lifetime Maximum)
- Repatriation (up to overall maximum limit)
- Intensive care (Usual, reasonable and Customary charges)
- Common Carrier Accidental Death ($50,000 per member(age 18 and older)$25,000 per member(age 18 and under))
- Accidental Death & Dismemberment (as per schedule of benefits; $250,000 Maximum benefit any one family)
- Terrorism Coverage ($50,000 Maximum Lifetime Limit, Eligible Medical Expenses only)
Remarks:
- PPO Network: Atlas America Insurance within the US uses the
Hygeia Network PPO network
- Renewal: Policies purchased online for 5 or more days are eligible for renewal. Policies can be renewed online before the expiration date of the plan. There is a $5 renewal fee. A new plan must be
purchased after 12 months of coverage.
- Cancellation: A full refund of premium is possible if a written request is provided prior to the start of the plan. If written
request is received after the Effective Date of coverage, the unused portion of the plan cost may be refunded minus a cancellation fee, provided no
claim has been submitted for reimbursement
