- Patriot America Plus Insurance provides a choice of Policy Maximum’s of $50,000, $100,000, $500,000
- Your choice of US$0, $100, $250, $500, $1,000 or $2,500
- Outside the U.S. and Canada:
For the Certificate Period, Patriot Travel Medical Insurance will pay 100% of Eligible Expenses after the Deductible up to the Overall Maximum Limit. - In USA:
In the PPO network, Patriot America plus Insurance pays 90% of eligible expenses up to US$5,000, then 100% up to the Policy Maximum. Outside the PPO network, the plan pays 80% of eligible expenses up to US$5,000, then 100% up to the Policy Maximum
- Patriot America Plus Insurance provides coverage for non U.S. citizens traveling outside their home country. It also provides optional coverage for preexisting condition medical and evacuation for ages less than 70.
- If the plan is purchased for a minimum of 30 days, coverage may be renewed (without break in coverage) for a total of up to two years.
- Patriot America Plus Insurance is underwritten by Sirius International Insurance Corporation.
- Patriot Travel Medical Insurance (Patriot America Insurance & Patriot International Insurance) is administered by International Medical Group, Inc.

Eligibilty
In order to be eligible and qualified for coverage under this insurance, a person must:
- complete and sign an Application as the Insured Person (or be listed thereon by proxy as an applicant and proposed Insured Person), and/or as the Insured Person’s Spouse and/or Dependent; and
- be an individual at least 15 days old and
- on the Effective Date and on subsequent renewals, must have legally departed the Home Country and legally entered the Host Country; and
- for Insured Persons 65 and older visiting the United States, the initial Period of Coverage must begin within 30 days of arrival in the United States or the individual must have valid international travel insurance coverage expiring within 30 days prior to initial Effective Date. United States domestic health care coverage does not serve to meet this requirement; and
- pay the required Premium on or before the Effective Date of Coverage; and
- receive written acceptance of his/her Application or extension from the Company; and
- not be Hospitalized, Disabled, or HIV+ on the Initial Effective Date; and
- not be a citizen of or have a permanent residence in the Host Country.

Remarks
- PPO Network: With the Patriot America plan, you may seek treatment with the hospital or doctor of your choice. When seeking treatment in the U.S., you may reduce your out-of-pocket costs by using the independent First Health Network PPO
- Renewal: Renewals are available in whole month or daily increments and may be completed online or by using a paper application, however, renewals of less than one month are available only online. For each renewal of less than one month completed online, you will be charged an additional US$5 processing fee.
- Cancellation: In order to be considered for a full refund, your request for cancellation for Patriot America Plus Insurance must be received prior to your effective date. If you do not have any claims filed, you may cancel your plan after your effective date, however you will be charged US$25 cancellation fee and only full month premiums will be considered for refunds. If you have filed claims, your premium is non refundable.
