| Policy Maximum
|
The Reside Worldwide
Medical Plan provides a Lifetime Maximum Benefit
of US$5,000,000 outside of the US and Canada
and $500,000 inside the United States and Canada. |
| Deductible
|
$70, $100, $250, $500, or $1,000
per injury/Sickness. |
| Co-insurance |
No co insurance; The Reside Worldwide
Medical Plan will pay up to pre defined limits
as defined in the Schedule of Benefits after
the Deductible per injury/sickness is paid.

| Coverage Options |
The Reside Worldwide Medical Plan
needs to be purchased to receive coverage annually
and can be annually renewed. |
| Plan Life
|
The Policy Period is for 12 months
and must be renewed annually as long as the
eligibility requirements are met and the renewal
premium is received. There are no additional
medical questions upon renewal. |
| Underwriter |
The Reside Worldwide Medical Plan
is underwritten by Lloyd's of London, member
of Lloyd's
Group of companies. |
| Claims Administrator
|
Seven Corners, Inc. administers
the Reside Worldwide Medical Plan. |
Eligibility:
The Reside Worldwide Medical Plan is available
to persons of any country who are at least 15 days
of age and not yet 75 years old, and will travel or
reside in the US or Canada for less than six months
out of the year.
For U.S. Citizens: Applicants
/ Insured Persons must either be outside the United
States at the time of application, or plan to depart
the United States within 30 days of the Certificate’s
Effective Date. In addition, the Insured Person must
reside outside the United States at least 6 months
during any given 12-month Policy Period in order to
be considered an Insured Person. If an Insured Person
resides in the United States longer than 6 months
during any given 12-month Policy Period, their coverage
shall immediately terminate.
For Non-U.S. Citizens:
Applicants / Insured Persons must be outside the United
States at the time of application, or plan to depart
the United States within 30 days of the Certificate’s
Effective Date. If the Insured Person is located in
the United States for more than 30 days after the
Effective Date and cannot obtain other health insurance,
a Proof of Eligibility Form must be submitted with
the Application.

The Reside Worldwide Medical Plan offers
a broad range of benefits including:

outside the US and Canada, $500,000 inside the US
and Canada;

in the Schedule of Benefits

per the Schedule of benefits

covered as per the Schedule of Benefits;

Evacuation coverage of $250,000 per person per policy
period;

The reside worldwide insurance can also provide assistance
with travel information and help with weather information,
lost passpert, exchange rate information, emergency
messages as well as interpreter referral
- PPO Network: Outside the US the
plan does not follow a network. You can go to any
hospital or doctor. In the US, the plan follows the Hygeia Network
