Assurant Student Select Insurance - Eligibility Requirements
Student Select Insurance is available to any eligible student. Any full-time college student*(taking nine or more college credits), attending a state accredited college or university, under the age of 30 and is able to answer "NO" to the medical questions on the application, is eligible for Student Select.
You must be a full-time student for a minimum of 31 days following the effective date of your policy.
* Assurant Health will accept the school's definition of full-time.
Assurant Student Select Insurance - How the Plan Works
The Student Select Insurance plan is simple, affordable and easy to obtain. You choose the plan that best meets your needs and budget.
Deductible Options: $250, $500, $1,000, and $2,500
Coinsurance (also referred to as Rate of Payment): 80/20 to $10,000

| You pay your calendar year deductible. |

| Once your deductible is satisfied, Assurant Health pays 80% of the next $10,000 of covered expenses. |

| Assurant Health pays 100% of your remaining covered expenses up to $100,000 for each illness or injury. Your total plan maximun is $1 million. |

Assurant Student Select Insurance - Preauthorization
If you are insured under a Student Select policy, you must obtain authorization for all hospital admissions, outpatient surgeries and certain other services. The number to call for Student Select preauthorization is 800-454-5105. The Student Select identification card, which is attached to the insurance contract you will receive, also lists the preauthorization phone number.
Assurant Student Select Insurance - No HMOs or PPOs
Since Student Select is not an HMO or PPO plan, you can visit the doctor or hospital of your choice. No referrals are needed, no non-network penalties are incurred ... the choice is yours!
Assurant Student Select Insurance - Payment Options
You can pay for your Student Select plan on an annual or semi-annual basis. We offer two convenient payment methods of credit card (Visa/MasterCard) or personal check. Both the annual and semi-annual payment options are available with the credit card payment method.
Assurant Student Select Insurance - Premium Refunds
If you are not 100% satisfied with the plan, you can return the contract within 10 days of delivery for a refund. If a cancellation request is received after the 10 day free look, a prorated refund will be provided as described in the contract. Simply put your request in writing and fax it to 414-299-6217 or mail it to: Assurant Health, P.O. Box 3176, Milwaukee, WI 53201-3176.
