Patriot Group Travel Medical Insurance
helps reduce the complications that arise from group
travel in foreign countries. The Patriot Group Travel
Medical Insurance provides complete medical benefits
for up to two years of overseas travel. Two plans
are offered tailored to either US citizens (Patriot
International® Group) or non-US citizens (Patriot
America® Group) planning travel outside of their
home countries.

|
The Patriot Group Medical
Travel Insurance provides Policy Maximum options
up to $2,000,000, and for those age 80 or older,
the maximum is $10,000. |
| Deductible
|
$0, $100, $250, $500, $1,000 or
$2,500 per Certificate Period. |
| Co-insurance |
- Outside the
U.S. and Canada:
The plan pays 100% of Eligible Expenses
after the Deductible up to the Overall Maximum
Limit.
- In USA :
Within the PPO, the plan pays 90% of eligible
expenses up to $5,000, then 100% up to the
Policy Maximum; outside of the PPO, the
plan pays 80% of eligible expenses up to
$5,000, then 100% up to the Policy Maximum.

| Coverage Options |
Coverage can be purchased in monthly
increments for up to 24 months, with renewal
available after three months of continuous coverage.
Optional Leisure Sports, Extreme Sports,
Patriot Return, and Chaperone/Faculty Replacement
riders. |

Insurance can be renewed monthly for up to 24
months. |

Eligibility:
-
The group member
and his/her traveling dependents must be US citizens
traveling outside the United States or non-US citizens
traveling outside their home country for a minimum
of 10 days up to a maximum of two years. For those
over 65 years old visiting the US, the initial Period
of Coverage must begin within 30 days of arrival
in the US; however, proof of previous valid international
travel insurance waives this requirement.

Open only to group
members, spouses, and unmarried children more than
14 days and less than 19 years old;
Coverage becomes effective on the latest of:

The date IMG receives
the completed Application Form and the appropriate
premium; ,

The date the group
member departs his/her home country; or,
-
The date requested
on the Application Form.
Coverage will end on the earliest of:

The end of the
period for which premium was paid;
-
The date requested
on the Application Form, or
-
The date the group
member returns to his/her home country.

Medical expenses
including Hospital Room and Board (Up to Policy
Maximum for average semi-private room rate), Outpatient
Medical, Local Ambulance, Emergency Room accident
and Emergency Illness;

Dental coverage
for injury due to accident or sudden pain;

Emergency evacuation
expenses up to the Policy Maximum;

Accidental Death
and Dismemberment coverage;

Emergency Reunion
coverage up to $15,000, and Return of Mortal Remains
coverage up to $25,000;

Return of Minor
Children and Political Evacuation coverage;

End of Trip Home
Country and Common Carrier Accidental Death Coverage;

Terrorism, Identity
Theft Assistance, Trip Interruption, and lost Luggage
coverage; and,
