and non-US (Patriot Executive America) citizen executive
traveler that provide comprehensive medical coverage
while outside of the Home Country. Let the Patriot

while you’re busy focusing on your business
overseas.

The Patriot Executive
annual travel insurance provides a Policy Maximum
of US$1,000,000; and for ages 70-75 the maximum
is US$50,000; |

| Co-insurance |
- Outside the
U.S. and Canada:
For the Certificate Period, Underwriters
will pay 100% of Eligible Expenses after
the Deductible up to the Overall Maximum
Limit.
- In USA :
Within the PPO, the plan pays 90% of eligible
expenses up to $5,000, then 100% up to the
Policy Maximum; outside of the PPO, the
plan pays 80% of eligible expenses up to
$5,000, then 100% up to the Policy Maximum.

The Period of Coverage is one year
from the effective date of the Certificate of
Coverage, and is non-renewable. |

Eligibility:
For US citizens and their spouses and
children, you must be under the age of 76, traveling
together outside of the United States frequently throughout
the year, and covered by an individual or group medical
plan.
For non-US citizens and their spouses
and children, you must be under the age of 76, covered
by an individual or group medical plan, and travel
together frequently outside of your home country throughout
the year.
Coverage begins one year from the effective
date of the Certificate of Coverage, and is non-renewable.
Coverage will end one year from the effective
date of the Certificate of Coverage, and is non-renewable.
Plan Benefits:
The Patriot Executive
medical plans offer a broad range of benefits including:

Coverage and benefits
for medically necessary charges that are for injuries
and illnesses for which treatment is first obtained
during the first 30 days of the trip;

Dental coverage
for injury due to accident or sudden pain;

Medical evacuation
expenses to the nearest qualified medical center
or hospital up to $25,000;

Emergency Reunion
coverage up to $15,000, and Return of Mortal Remains
coverage up to $25,000;

Return of Minor
Children and Political Evacuation coverage

Terrorism, Identity
Theft Assistance, Trip Interruption, and lost Luggage
coverage; and,

Pre-existing Conditions
coverage up to $5,000 per period of coverage.
Remarks:
- PPO Network: Outside the US the
plan utilizes the on-line International Provider
Access database to assist in locating health care
providers. Inside the U.S. the plan members can
avail the First
Health network (login: img).
- Cancellation: Full refund is
available if the cancellation request is received
in writing before the start of the plan.
- Maximum Trip Duration: 30 days
Although the Patriot Executive plan is not renewable,
it can be rewritten for succeeding or subsequent
coverage periods.
