- Outside the
U.S.:
After the deductible is paid, the program
pays 100% to the selected Medical Maximum.
- In USA:
After the deductible is paid, the program
pays 80% of the next $5,000 of eligible
expenses, then 100% to the selected Medical
Maximum.

| Coverage Options |
The minimum period
of coverage under Liaison® Worldwide is
five (5) days, maximum is twelve (12) months.
Coverage can be purchased in a combination
of monthly and/or daily periods by paying
the appropriate plan cost. |
| Plan Life
|
The plan can be purchased for
a minimum of 5 days to a maximum of 12 months
at any given time and renewed up to 3 years
(1 year for persons age 65 and over). |

underwritten by Nationwide Mutual Insurance
Company, Nationwide Life Insurance Company and
Nationwide Mutual Fire Insurance Company. |

Eligibility:
Liaison® Worldwide provides coverage
for individuals and families (including unmarried
dependent child(ren) over 14 days and under 19 years
of age)while traveling outside of their Home Country.
Plan Benefits:
Liaison Worldwide Insurance offers a broad range of
benefits including:
- In-patient hospitalization, surgery, medical diagnostic
expenses (up to policy maximum)
- Out-patient doctor office expenses (up to policy
maximum)
- Prescription Drugs (up to policy maximum)
- Emergency Evacuation/repatriation(up to $300,000
in addition to policy maximum)
- Return of Mortal Remains (up to $50,000)
- Intensive care (URC up to policy maximum)
- Common Carrier Accidental Death ($50,000 per adult,
$25,000 per child(ren) under age of 18; $250,000
Maximum per family)
- Accidental Death & Dismemberment ($25,000 Principal
Sum for Insured or Insured Spouse, $5,000 for Dependent
Child(ren))
- Terrorism Coverage (URC up to policy maximum)

PPO Network:
Liaison Worldwide Insurance within the US uses the
Hygeia
Network PPO network and outside the US one can
use any provider.

Renewal: Policies
purchased online for 5 or more days are eligible
for renewal . Policies purchased via fax or mail
can be extended if purchased or renewed for 3 months
or more. There is a $5 renewal fee.

Cancellation:
A full refund of premium is possible if a written
request is provided prior to the start of the plan.If
written request is received after the Effective
Date of coverage, the unused portion of the plan
cost may be refunded minus a cancellation fee, provided
no claim has been submitted for reimbursement.
