Liaison International insurance provides
coverage for US as well as Non US Citizens. Liaison International
Insurance offers medical insurance and emergency services
to individuals and families (including dependent child(ren)
over 14 days and under 19 days of age) travelling
outside their Home Country. Liaison insurance for
non US citizens as well as Us citizens provides worldwide
travel medical insurance for hospitalization, Intensive
care, surgery as well as prescription drugs.

| Co-insurance |
- Class 1: U.S. or Canadian citizen traveling outside the United States:
After You pay the Deductible, the plan pays 100% to the selected Medical Maximum.
- Class 2: Non U. S. citizens traveling to the United States.
Option 1: After You pay the Deductible, the plan pays 80% of the next $5,000 of eligible
expenses, then 100% to the selected Medical Maximum
Option 2: After You pay the Deductible, the plan pays 100% of the next $2,500 of eligible
expenses, then 80% to the selected Medical Maximum.

| Coverage Options |
Liaison International provides optional covergae for Hazardous Sports like
motorcycle/motor scooter riding (whether as a driver or passenger), hang gliding, parachuting,
bungee jumping, water skiing, snow skiing, snowmobiling, snowboarding and spelunking when the
appropriate premium has been paid. |
| Plan Life
|
The minimum Period of Coverage under the Liaison® International Plan is five (5) days,
maximum is six (6) months. Coverage can be purchased in a combination of monthly and/or daily periods
by paying the appropriate Plan Cost.

Liaison® International is underwritten by Nationwide Mutual Insurance Company,
Nationwide Life Insurance Company and Nationwide Mutual Fire Insurance Company. (States not
underwritten by Nationwide are underwritten by Certain Underwriters at Lloyd’s of London.) |

Eligibility:
Liaison® International Eligible individuals may also purchase coverage for their
eligible dependents. An eligible spouse shall be defined as the Primary Insured’s legal spouse.
An Eligible Dependent Child shall mean the Primary Insured Person’s unmarried child(ren) over
fourteen(14) days and under nineteen (19) years of age.

Your coverage begins on the latest of the following:
- The date and time the Company receives a completed application and Plan Cost for the Period of Coverage; or
- The Effective Date requested on the application; or
- The moment You depart Your Home Country; or
- The date the Company approves the application.
Your coverage terminates on the earlier of the following:
- Your return to Your Home Country (except as provided under the Home Country Coverage); or
- The expiration of six (6) months from the Effective Date of Coverage; or
- The date shown on the ID card; or
- The end of the period for which Plan Cost has been paid; or
- The date You fail to be considered an Eligible Person; or
- The maximum benefit amount has been paid.

Remarks:

PPO Network: Liaison
International Insurance within the US uses the
Hygeia
Network PPO network and outside the US one
can use any provider.
- Renewal: Policies purchased
online for 5 or more days are eligible for renewal and can be renewed
online.
Policies purchased via fax or mail can be extended
if purchased or renewed for 3 months or more.There
is a $5 renewal fee.
- Cancellation: A full refund
of premium is possible if a written request is provided
prior to the start of the plan. If written request
is received after the Effective Date of coverage,
the unused portion of the plan cost may be refunded
minus a cancellation fee, provided no claim has
been submitted for reimbursement
